Notify the enrollment broker that you want to apply for services. They will begin by scheduling your initial in-home assessment visit.

You’ll be given a checklist of documents to gather for that visit—review it carefully and make sure everything is ready.

PA IEB will also send your primary care provider a Physician Certification Form to complete and return.

Schedule Your First Visit

 During the visit, a PA IEB representative will discuss your needs and abilities with you.
You will complete your Medicaid application and any other required forms.
After your Medical Assistance (MA) application is submitted, your local County Assistance Office (CAO) will review it and may send a letter requesting additional forms or information. 

What Happens Next

After your first visit, the IEB will notify your local Area Agency on Aging (AAA) and arrange for them to schedule an in-home needs assessment with you. This assessment is called the Functional Eligibility Determination (FED).

Once completed, the assessment results will be sent to PA IEB for review.

A review of your Physician’s Certification and Functional Assessment of Need takes place

 

The PA IEB must receive both your Functional Eligibility Determination (FED) and the Physician Certification Form within 86 days of your first in-home visit.

If both documents are received on time and you meet the clinical program eligibility requirements, your application will be forwarded to the County Assistance Office (CAO).

The CAO reviews your waiver application

The County Assistance Office (CAO) will decide whether you meet the financial requirements for waiver services. If any information is missing, they will mail you a letter explaining what’s needed and how to submit it.

Once your application is complete, the CAO will issue a decision to approve or deny it.
